Picture identification (driver’s license, state identification card, passport)
Insurance card, as issued from your carrier
Patient Forms, completed
Form of payment (cash, check, credit card). We accept MasterCard, Visa and Discover.
Current medications bottles or list
Medical records, as issued from prior physicians and/or hospitals
