Dr. Esther Yoon is a designated civil surgeon fully certified by the United States Department of Homeland Security and the United States Citizenship & Immigration Services to provide civil medical examinations. 

As an applicant, you are required to file Form I-693 approved by a designated civil surgeon to establish that you are not inadmissible to the United States on public health grounds.

Prior to your appointment, be sure to prepare and bring the following with you:

• Form I-693, Report of Medical Examination and Vaccination Record: as required by the U.S. Citizenship & Immigration Services, please download and print Form I-693 below. Complete Part 1 only and submit to reception upon checking in. Do not complete any other part of Form I-693 besides Part 1. For your convenience, the instructions for completing Form I-693 that you may also download are also provided below.
  

• Government issued photo identification: a valid and unexpired passport or driver’s license. If you are 14 years of age or younger, bring identification indicating your name, birth date, place of birth and your parent’s full name. Acceptable forms of identification include your birth certificate (with English translation, if necessary) or an affidavit.

• Insurance card: current proof of coverage, as issued from your carrier.
  

• Vaccination and immunization records: such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease. Documentation must be on recognized official medical letterhead or forms. USCIS guidelines dictate that e-mails documenting vaccinations are insufficient and do not meet this requirement
  

• Form of payment: the USCIS civil medical exam fee covers a civil medical examination fully comporting with USCIS requirements and any necessary follow up visits where we present you with your processed application. When scheduling your appointment, reception will advise you as to the fees.

During your visit, Dr. Yoon will review your medical history and records to determine if you require any vaccinations or additional lab work.

The USCIS examination fee is not inclusive of potential vaccines, additional labs or X-rays that may be required.

The processing time for your USCIS application is dependent upon the timeliness of your receiving any requisite vaccinations, lab work and/or x-ray tests and seeing that the results are promptly submitted to us.

To expedite your application, you can perform required labs by the USCIS for syphilis, gonorrhea, and tuberculosis (Quantiferon Gold testing) prior to your exam. These results and chest X-ray, if required, must be performed within one year of the USCIS exam date. (Please review the acceptable types of tests for each disease as noted in the USCIS form.) If you do not have a physician these tests can be ordered for you during your USCIS exam. Additionally, you should have your immunizations updated or have proof by labwork titers or present documentation of prior immunizations for an expedited application. 

Upon fully completing the civil medical exam’s requirements, we will process the approved Form I-693 and submit the approved form to you in an official sealed envelope for you to submit to USCIS. We also provide you with a courtesy copy that we strongly suggest you retain for your personal records. Please do not attempt to open, alter or tamper with the sealed envelope as the USCIS will not accept it and will return it to you.
